Authorities identified the victim as Scott W. Shoemaker, 38, who now lives in Black River Falls.

Three of Shoemaker’s children in the vehicle, who all live in River Falls, were seriously injured. They were all initially transported by ambulance to Regions Hospital. Region officials said they were later transferred to Gillette Children’s Hospital. Officials at Gillette would not release any information on the children’s conditions.

The injured children are Nicole, 12, Brock, 14, and Mariah, 16, Shoemaker. Nicole and Brock attend Meyer Middle School in River Falls and Mariah attends River Falls High School.

Injuries suffered by two of the children were described as "life-threatening" on the day of the accident by the State Patrol which assisted St. Croix County deputies at the scene.

In a communication from River Falls Superintendent Jamie Benson, he said the “family recently moved back to the River Falls School District after living in Black River Falls the past few years.”

The crash, at the intersection of Old Hwy. 35 and Tower Road, was reported at 4:04 p.m., Saturday. Hudson EMS and fire responded, along with an ambulance from River Falls. All occupants of the car were said to be unconscious while the van's driver appeared uninjured.

Officers initially requested two medical helicopters to the scene but the aircraft were later cancelled and the three children were taken directly to Regions by ambulance.

The driver of the van, Jason D. Edwards, 42, of Hudson, was transported to Hudson Hospital to be checked but the Patrol said he was treated and released.

Authorities said Shoemaker was westbound on Tower Road when he failed to obey a stop sign at Old Hwy. 35 and traveled into the path of the northbound van.

Online court records indicate Shoemaker lived on Kennedy Street in River Falls as recently as 2007. According to the online records, he was divorced in 2001.

Everyone involved in the crash was wearing seat belts. The crash remains under investigation. The fatality was the 10th recorded this year on St. Croix County roads.
