VILLAGE OF NORTH HUDSON: RESOLUTION NUMBER 2008-12

RESOLUTION TO ADOPT A DENTAL COVERAGE PLAN RENEWAL FOR ELIGIBLE VILLAGE EMPLOYEES

The Board of Trustees of the Village of North Hudson, Wisconsin, at a meeting held this 6th day of November, 2008, hereby adopts the following resolution:

WHEREAS, the Village of North Hudson provides dental insurance coverage for eligible employees, and;

WHEREAS, the Village of North Hudson has received a renewal quote for said coverage from Delta Dental Premier effective 01-01-2009, and;

WHEREAS, the renewal quote provides for no increases in premium amounts and provides for the same coverage as presently available under the plan;

NOW, THEREFORE, BE IT RESOLVED that the Village of North Hudson renews the dental insurance program for eligible employees as follows:

1. Effective 01-01-2009, Dental coverage for eligible Village of North Hudson employees is renewed with Delta Dental Premier.

2. Premiums and coverage will remain at the same levels as they were in the year 2008.

3. The single deductible will remain at $50.00 and the family deductible will remain at $150.00. Annual maximum benefits are capped at $1,000.00 and the orthodontic maximum is $1,500.00.

4. Premiums will be as follows:

Employee Only: $27.34

Employee and Spouse: $53.87

Employee and Children: $57.69

Full Family: $99.52
